If you have an urgent medical problem outside office hours please contact your primary care physcian. For emergencies dial 911 or go to the closest emergency room.

Medical Records & Forms

Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization from you.

Prescriptions:

Prescription refills and follow-up office appointments should be done during routine business hours. Please ask your pharmacist to fax the prescription refill request to the phyisican's office (see listing above). Every effort will be made to process your prescription request within a 48 hour period. Controlled substances such as narcotics will not be authorized after business hours. If the prescription is mail order, please allow plenty of time for our offices to process the request.
